8/2/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax verbalized to QMHPA that she was doing well. QMHPA informed Ms. Broadnax that this session was going to be conducted out in the community to assist her with positive interactions with others surrounding her to reduce her symptoms of anxiety and depressive symptoms. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHPA engaged Ms. Broadnax in a discussion surrounding her medication management. QMHPA guided Ms. Broadnax in interacting with others throughout the community. QMHPA assisted Ms. Broadnax with being more social able. QMHPA discussed with Ms. Broadnax how laying around and not engaging in social activities with others in the community or her family, will cause her become isolated and not interacting with people. QMHPA engaged Ms. Broadnax in a skill building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to assist her with recognizing triggers and questioned her about situations that causes her to become distressed. QMHPA discussed with Ms. Broadnax when she becomes occupied with negative thoughts, she needs to occupy herself with a constructive activity. QMHPA assisted Ms. Broadnax with a brainstorming exercise where she come up with activities, she finds enjoyable. QMHPA guided Ms. Broadnax with imagining exercise where she is to think of her being the only person on the earth and think of anything activity she would enjoy. QMHPA encouraged Ms. Broadnax to be able to use her imagination to help assist with her depression as well as displaying a positive attitude. Response: Ms. Broadnax appeared to be receptive to QMHPA’s intervention and services that were provided to her during the session. Ms. Broadnax discussed with QMHPA that she likes to go to the park and walking. Ms. Broadnax verbalized to QMHPA that she walks to calm her nerves at times when she is angry or depressed. Ms. Broadnax discussed with QMHPA that she thinks back to when she was growing up or something she seen on television when she laughs out loud as a way to occupy herself with a constructive activity. Ms. Broadnax said if she was the only person on earth she would eat a lot of pizza, stay at the theme park. Ms. Broadnax was able to entertain the thought of imagination.
